To obtain a copy of your consumer report , please download the Report Request Form using the link above, complete and return it to us via fax or US Postal Service. The form will not be considered complete unless you have signed your name in the designated place and included a copy of your Driver's License.
Please note that we cannot inform you of the specific reasons your application for credit was denied. We can only supply you with the information given to our client at the time of your application.
A consumer credit report is a factual record of an individual’s credit payment history as reported by his or her creditors. It can be provided only for purposes permitted by law and serves as your credit references for businesses. Credit reports are most frequently used to help lenders quickly and objectively decide whether to grant you credit. Once a consumer credit report is requested, the information requested is compiled in a matter of seconds and transmitted back to the lender for their consideration. Some examples of lenders include car loans, credit cards, rent-to-own, short – term/unsecured loans, community banks and credit unions. A credit report may also be used in making decisions about employment, rental agreements, insurance applications and other business relationships.
A typical credit report will include specific information about the consumer that will help a lender to assess their risk associated with lending. There are also many regulatory safeguards in place to prevent discriminatory lending practices. Some of the most common information included in a credit report will include:
Identification: your name, current and previous addresses, telephone number, reported variations of your social security number, date of birth, employer and your spouse’s name.
This information is gathered by the credit bureaus through the consumer’s application process, so it’s accuracy is dependent on the consumer’s ability to fill in the forms completely and clearly with correct information each time they apply for credit.
The federal government also includes a geographic code for the area in which a consumer lives in each report. This geographic code is used to prevent discriminatory lending practices based on the area in which a consumer resides.
Account history: specific information about each credit account and/or banking account you hold including the date that the account was opened, credit limit or loan amount, balance, monthly payment, payment status and payment history.

Inquiries: those people who have viewed your information. This information comes from the credit reporting agency.
